- For continuity of care: We may provide your information to government bodies or licensure boards for HIPAA-compliancy audit per the limitations and guidelines outlined in HIPAA and detailed in our HIPAA notice of policy practices; to third-party applications and business partners that provide services and products under the required HIPAA-compliant business associate agreement to protect your privacy; or to a records custodian in the event of our incapacitation or death. 
- To manage your requests: To attend and manage your requests to us.
- To provide you with news, general information, and special offers about other goods, services and events which we offer that are similar to those that you have already purchased or enquired about unless you have opted not to receive such information.

- When you voluntarily choose to interact in public areas with other users, directly or indirectly, such as on social media sites associated with this company and embedded on this website. - For other purposes: We may use your information for other purposes, such as data analysis, identifying usage trends, determining the effectiveness of our promotional campaigns and to evaluate and improve our service, products, services, marketing and your experience.

Therapy client data containing protected health information will be retained per legal guidelines. The company will also retain usage data for internal analysis purposes. Usage data is generally retained for a shorter period of time, except when this data is used to strengthen the security or to improve the functionality of our Service, or We are legally obligated to retain this data for longer time periods.

Transfer of Your Personal Data

If such breach involves protected health information, we will comply with the breach notification rule to notify you and the Department of Health and Human Services Secretary that a breach has occurred. 

Contact Policy
We would like to communicate with you using electronic communication (e.g., phone, text, email). Please remember electronic communications such as voicemails, texts or emails are unencrypted and may be neither secure nor confidential.If you have a significant issue that you wish to discuss with us, please DO NOT use email or text; rather, please leave a detailed voicemail message for us, and we will get back to you as soon as possible. If it is an emergency or you are in a crisis, please call 911. We place a high priority on getting back to our clients in a timely fashion. Therefore, if you do not hear back from us in at least 24 hours, then we probably didn't get the message. Unfortunately, phone systems periodically have problems, so please keep that in mind and kindly call again.

During the course of treatment, it may be useful for us to communicate with you electronically, for example, via email or text messages. The benefits of electronic communication can include: (1) resolving scheduling and billing issues quickly and efficiently (2) allowing your therapist to send reminders and appointment confirmations (3) transmission of helpful resource material, worksheets, educational information, etc. (4) access to your therapist remotely in between scheduled sessions, if you and your therapist have determined that this is clinically appropriate

We strive to provide you with convenient, timely means of communicating with us, and also make every effort to protect your privacy; however, electronic communication is never completely secure. Some potential risks of electronic communication include: (1) others accessing your device in case of loss/theft, or incidental contact at home/work (2) email accounts can be hacked (3) text messages are stored on servers (4) e-mails or text messages may be delivered to an incorrect address

To receive electronic communications, we require a signed Consent for Electronic Communication form on file. This assures:
- you have been informed of the above risks of transmitting my protected health information by unsecured means and understand these risks and the potential benefits of communicating electronically with Create Outcomes administrators and clinicians.
- you understand that you are not required to sign this agreement, and can terminate this consent at any time
- email, text messaging, or any other forms of electronic communication should not be used in an emergency situation
- you grant permission to Create Outcomes to email/text me by unencrypted means (sent without a passcode, which is less secure than the communication would be if it were encrypted, that is, sent with a passcode)
- the practice makes available to me the other means of communication (phone calls, secure patient portal messages, physical mail) that are designed to be more secure and to maintain confidentiality

Children's Privacy
This company serves adults 18+ only. As such, the service does not address anyone under the age of 13 and does not knowingly collect personally identifiable information from anyone under the age of 13. However, this is a public website. Parents and guardians are responsible for the use of this public website by their minor children.
